Carlmont HS Athletic Clearance Information

PLEASE NOTE the dates on the site are for all sports and students who wish to play football, need to be cleared BEFORE August X to receive their equipment on August X. The Carlmont Athletics clearance information can be found here: https://carlmontathletics.org/athletic-clearance-information Sequoia Union High School District utilizes an online registration and clearance process for athletic eligibility. Click the link below to access Sportsnet: *New parents must create a login with a username and password* In addition to completing the online registration, a completed physical exam form must be either uploaded to SportsNet or turned in to Ms. Eggli in the main office. Physicals are good for one calendar year from the date of the last physical. It is imperative that the date of physical exam on the top right of page of two of the physical form is filled in. Online registration needs to be completed once per school year. Allow 5 days for processing once the online registration and physical form have been submitted. *Players need to be registered on Sportsnet and have a completed physical to receive equipment and be able to practice starting in August* |
